RESOLUTION AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE DEPARTMENT OF REHABILITATION AND AUTHORIZING THE DIRECTOR OF THE HUMAN SERVICES AGENCY TO SIGN SUBSEQUENT AMENDMENTS FOR THE PURPOSE OF MAKING MINOR MODIFICATIONS AND TO ACCEPT INCREASES TO THE AGREEMENT NOT EXCEEDING AN AGGREGATE OF $25,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, a new Agreement has been issued by the Department of Rehabilitation for the purpose of funding situational assessment, work adjustment, employment preparation, job development and placement services for County Mental Health consumers in the amount of $767,190, which provides funding the Human Services Agency for FY 2004-05 services only, with funding for future years in the form of Amendments, and the term of said Agreement is July 1, 2004 through June 30, 2007; and

WHEREAS, the Human Services Agency and the Department of Rehabilitation have successfully worked together since 2002 to provide such services to County Mental Health consumers; and

WHEREAS, this Board has been presented with a form of this Agreement and has examined and approved said Agreement as to both form and content and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER DETERMINED AND ORDERED that the Director of the Human Services Agency shall be authorized to execute subsequent amendments for the purpose of making minor modifications and to accept increases not to exceed an aggregate of $25,000.
