November 29, 2004

December 14, 2004

Honorable Board of Supervisors

Charlene A. Silva, Director of Health Services

Gale Bataille, Director of Mental Health Services

Agreement with Anthony Thekkek, dba Burlingame Hacienda

Adopt a resolution:

A) Authorizing the President of the Board to execute an agreement with Anthony Thekkek dba Burlingame Hacienda and

B) Authorizing the Director of the Health Services Agency to execute subsequent amendments and minor modifications not to exceed an aggregate of $25,000 and to make minor changes in the type of services and activities provided under the Agreement.

Since December 17, 1985, your Board has approved resolutions authorizing expenditures for residential care facilities serving mentally ill clients.

Consistent with the Mental Health Services Division Plan, contracts are offered to any qualified provider for these residential services. As required in this process, a public notice will be published every year to solicit applications for new providers.

This contractor operates a 17-bed board and care facility for difficult to place clients. This contractor provides two (2) levels of service, the Step Down Service Level and the Augmented Service Level. Clients at the Step Down Service Level are generally more independent and stable, requiring lower levels of supervision and less intensive services. The Augmented Service Level is for seriously mentally ill clients who require a higher level of supervision and services including more intensive mental health services, social skills training and assistance with activities of daily living.

This program was the subject of scrutiny last spring due to concerns raised by county unions about other out of county facilities operated by this contractor. As a result of those concerns, Mental Health conducted a special review of the program and found the quality of care to be appropriate. In addition, Mental Health continues to monitor the program and treatment of residents through this facility on at least a weekly basis. This program meets the needs for older adults who are severely mentally ill. Failure to renew this agreement would lead to a serious gap in the availability of placement resources for these clients.

The performance objective is as follows:

Performance Measure

2003-04 Actual

Minimum percentage of client survey respondents who shall rate services as good or better.*

* Data was not collected in FY 2003-04. The State DMH does not require satisfaction collection for these non-clinical residential services. Mental Health will collect baseline data on this measure using a new collection tool for FY2004-05.

Vision Alignment

This Agreement with Anthony Thekkek dba Burlingame Hacienda, keeps the commitment to Ensure Basic Health and Safety for All and goal number 8: Help vulnerable people, the aged, disabled, mentally ill, at risk youth, and others - achieve a better quality of life. Fiscal Impact

The term of the Agreement is July 1, 2004 through June 30, 2007. The Agreement has been reviewed and approved by County Counsel and Risk Management.

The Agreement maximum is $151,890 for the three-year term. The anticipated Agreement maximum for FY2004-05 is $50,630. Sales tax provided through realignment will cover $45,061 of the cost. The remaining $5,569 is net county cost, as maintenance of effort for realignment. A similar arrangement is in place for FY2005-06 and FY 2006-07.
