RESOLUTION AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH HORIZON SERVICES, INC.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Human Services Agency (HSA) accepted a five-year grant from Department of Health and Human Services (DHHS), Center for Substance Abuse Treatment (CSAT), grant number 1 H79 TI 16683-01 to provide substance abuse treatment for the homeless in the amount of $1,991,088 for the period of 9/30/2004 to 9/29/2009; and

WHEREAS, On December 7, 2004, the Board of Supervisors will adopt a resolution accepting CSAT Grant Number 1 H79 TI 16683-01, and waive the Request for Proposals (RFP) process and authorize the Alcohol and Other Drug Services to initiate five- year contracts with the service providers designated in the grant proposal including the Horizon Services, Inc. for the purpose of providing substance abuse treatment and prevention services in the amount of $1,000,000; and

WHEREAS, This Board has been presented with a form of such Agreement and said Board has examined and approved same as to both form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
