RESOLUTION (1) AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE

AN AGREEMENT WITH THE CALIFORNIA DEPARTMENT OF HEALTH SERVICES TO PROVIDE HIV SERVICES FOR THE TERM OF JULY 1, 2004 TO JUNE 30, 2007 FOR A TOTAL OBLIGATION NOT-T0-EXCEED $4,186,170; (2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO SIGN MEMORANDA OF UNDERSTANDING FOR THE PROGRAMS RELATED TO THE AGREEMENT; (3) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR

MODIFICATIONS NOT-TO-EXCEED $25,000 PER AMENDMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with the California Department of Health Services (CDHS) reference to which is hereby made for further particulars; and

 

WHEREAS, San Mateo County AIDS Program will provide early intervention, HIV prevention and education, high risk testing and outreach, case management and AIDS surveillance services for the period of July 1, 2004 through June 30, 2007, for a total obligation not to exceed $4,186,170; and

 

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Agreement as approved by the County Manager and County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to sign memoranda of understanding for the programs related to the Agreement.

 

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to execute minor amendments and modifications to the Agreement not-to-exceed $25,000 per amendment.
