BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION ACCEPTING A GRANT AWARD FROM THE PENINSULA COMMUNITY FOUNDATION IN THE AMOUNT OF $40,000 TO FUND A PROGRAM MAKING COMPREHENSIVE DENTAL CARE AVAILABLE TO LOW-INCOME PATIENTS OF SAN MATEO COUNTY.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 3, 2004, the Dental Department at San Mateo Medical Center, under the supervision of Dr. Ann Marie Silvestri, submitted a proposal to the Peninsula Community Foundation to fund a program that would make comprehensive dental care available to low-income patients of San Mateo County; and

WHEREAS, On November 30, 2004, Dr. Silvestri was notified that the proposal had been favorably reviewed and that the award would be granted in the amount of $40,000 for the period December 1, 2004 to November 30, 2005; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an grant award, reference to which is hereby made for further particulars, whereby the Peninsula Community Foundation will allocate funds in the amount of $20,000 for FY2004-05 and $20,000 for FY2005-06 for the provision of a program making comprehensive dental care available to low-income patients of San Mateo County; and

WHEREAS, this Board has been presented with a form of the award and has examined and approved it as to both form and content and desires to accept the award:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to accept said award and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
