RESOLUTION AUTHORIZING EXECUTION OF 1) AN AMENDMENT TO THE PREVIOUS AGREEMENT WITH BLOOD CENTERS OF THE PACIFIC AND
2) A NEW AGREEMENT WITH BLOOD CENTERS OF THE PACIFIC

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on March 16, 2004, the Board of Supervisors, by Resolution Number 66547, authorized an Agreement in the amount of $609,000, for the term January 1, 2004 to December 31, 2004, for the purpose of supplying blood, blood components and blood derivatives to San Mateo Medical Center; and

WHEREAS, San Mateo Medical Center’s need for blood, blood components, and blood derivatives has increased over the past year, increasing the monthly average to $56,600 thereby depleting the maximum amount of the previous Agreement; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a new Agreement, reference to which is hereby made for further particulars, whereby Blood Center of the Pacific shall supply blood, blood components and blood products, for the period January 1, 2005 to December 31, 2005; and

WHEREAS, the parties now want to amend the previous Agreement by increasing the amount of the Agreement by $70,000 changing the Agreement obligation from $609,000 to $679,000; and

WHEREAS, both parties now wish to enter into a new Agreement, to supply blood, blood components and blood products for the period January 1, 2005 through December 31, 2005, for a total obligation of $708,000; and

WHEREAS, this Board has been presented with a form of the Amendment and the Agreement and has examined and approved the Amendment and the Agreement as to both form and content and desires to enter into the Agreement:

NOW THEREFORE, IT IS HEREBY RESOLVED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment and said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
