RESOLUTION AUTHORIZING THE EXECUTION OF AMENDMENT 1 TO AGREEMENT NO. AP-0405-07 WITH THE CALIFORNIA DEPARTMENT OF AGING FOR TITLE III/VII OF THE OLDER AMERICANS ACT OF 1965 FOR FISCAL YEAR 2004-2005

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on July 27, 2004, the Board of Supervisors, by Resolution Number 66823, authorized an Agreement in the amount of $3,060,693 for the term of July 1, 2004 through June 30, 2005, for the provision of a nutrition and supportive services program;

WHEREAS, the parties now wish to amend the Agreement by increasing the contract amount by $488,192 changing the agreement amount from $3,060,693 to $3,548,885 and amending the scope of work with this change for the purpose of enhancing services to the vulnerable people served; and

WHEREAS, the Board has been presented with Amendment 1 and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said Amendment 1 to Agreement No. AP-0405-07 for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President’s signature thereto.
