RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT WITH DISCHARGE RESOURCE GROUP FOR PHYSICAL AND OCCUPATIONAL THERAPY SERVICES FOR MEDICALLY ELIGIBLE CALIFORNIA CHILDREN SERVICES CLIENTS IN THE AMOUNT OF $339,000 FOR THE TERM JULY 1, 2004 THROUGH JUNE 30, 2007

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an agreement with Discharge Resource Group to provide pediatric occupational and physical therapists as needed for California Children Services Program, reference to which is hereby made for further particulars; and

WHEREAS, the term of said agreement is July 1, 2004 through June 30, 2007, and the total obligation shall not exceed $339,000; and

WHEREAS, the Board of Supervisors has examined and approved the same as to form and content and desires to enter into said Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
