RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH GRAHAM BEAUMONT, M.D. TO INCREASE THE AGREEMENT MAXIMUM BY $50,000 TO A TOTAL OBLIGATION OF $150,000 AND AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 IN THE AGGREGATE AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 8, 2003, the County Manager, approved Agreement Number 61000-03-C197 in the amount of $100,000 for the term of July 1, 2002 to June 30, 2005 for the purpose of providing psychiatric inpatient and outpatient professional mental health services to San Mateo County Medi-Cal, HealthWorx, Healthy Families and Healthy Kids clients and to uninsured clients for whom the County has accepted responsibility; and

WHEREAS, the parties wish to Amend the Agreement by increasing the amount by $50,000 changing the Agreement obligation from $100,000 to $150,000 due to a critical shortage of psychiatrists available and willing to contract with the County and high level of need for psychiatric services by San Mateo County clients in the community; and

WHEREAS, the Board is presented with this Amendment and desires to enter into same;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, THAT the Director of the Health Department is hereby authorized to execute subsequent amendments and minor modifications not to exceed $25,000 in the aggregate and to make minor changes in the types of services and activities provided under the Agreement.
