RESOLUTION A) WAIVING THE REQUEST FOR PROPOSAL (RFP) PROCESS, B) AUTHORIZING EXECUTION OF AN AGREEMENT WITH TRILOGY INTEGRATED RESOURCES LLC TO MAINTAIN THE STATE-OF-THE-ART NETWORK OF CARE WEB SITE FOR AGING AND ADULT SERVICES AND MENTAL HEALTH SERVICES FOR MAY 1, 2005 THROUGH JUNE 30, 2006, AND C) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES TO THE TYPES OF SERVICES AND ACTIVITIES PROVIDED

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, the best interest of the County of San Mateo would be served by waiver of the Request for Proposal, in that Trilogy Integrated Resources LLC is considered a sole-source provider of the state-of-the-art Network of Care web site;

 

WHEREAS, the parties wish to enter into an Agreement, to provide for the state-of-the-art Network of Care web site, for the period of May 1, 2005 through June 30, 2006, for a total obligation of $121,600; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, an Agreement between the County of San Mateo and Trilogy Integrated Resources LLC, reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

IT IS HEREBY FURTHER RESOLVED that the Request for Proposal process is hereby waived.

 

BE IT FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications to this Agreement during the term of the Agreement, not to exceed $25,000 and to make minor changes in the types of services and activities provided under the Agreement.
