RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH THE PENINSULA HEALTHCARE DISTRICT INCREASING THE AMOUNT BY $682,250 TO $1,432,250 AND EXTENDING THE TERM TO JANUARY 1, 2003 – DECEMBER 31, 2005

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, The Board of Supervisors, by resolution number 65792 authorized an agreement with the Peninsula Healthcare District in the amount of $750,000 for the term January 1, 2003 – December 31, 2004 for funding for the Children’s Health Initiative

WHEREAS, there has been presented to this Board of Supervisors for its consideration and approval, an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the Peninsula Healthcare District will continue to support the Children’s Health Initiative by funding health insurance benefits for children from age 6 through age 18 years under a contract with the County of San Mateo; and,

WHEREAS, the parties wish to enter into an amendment to the agreement to provide funding for the Children’s Health Initiative increasing the amount by $682,250 to $1,432,250 and extending the term to January 1, 2003 – December 31, 2005; and,

WHEREAS, this Board has been presented with this Amendment and has approved it as to both form and content, and desires to enter into said Amendment;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
