RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH

CALIFORNIA ADVANCED IMAGING MEDICAL ASSOCIATES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 2, 2002 the Board of Supervisors, by Resolution 65162, authorized an Agreement in the amount of $4,125,000 for the term April 1, 2002 through March 30, 2005 for the purpose of providing radiology services to the San Mateo Medical Center; and

WHEREAS San Mateo Medical Center’s has a continued need for radiology services, and

WHEREAS, both parties now wish to enter into an Agreement to provide radiology services that supercedes Resolution 65162 for the period April 1, 2004 through March 30, 2007 for a total obligation of $5,456,815; and

WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby California Advanced Imaging Medical Associates shall continue to provide professional radiology services; and

WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the amendment:

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
