RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH ASSOCIATED MEDICAL SPECIALISTS (AMS)
FOR INTERNAL MEDICINE SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on April 2nd 2002 the Board of Supervisors, by Resolution 65161, authorized an Agreement in the amount of $9,913,512 for the term April 1, 2002 through March 31, 2005 for internal medicine services, including cardiology, to the San Mateo Medical Center; and

WHEREAS, the San Mateo Medical Center requires uninterrupted internal medicine hospital services during the interim period of contract negotiations; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Associated Medical Specialists (AMS) shall provide internal medicine services, including cardiology, starting April 1, 2005; and

WHEREAS, the parties now wish to amend the previous Agreement by extending the length of the contract by six months from April 1, 2005 to September 30, 2005 with an increase in reimbursement of $1,570,002 from $9,913,512 to $11,483,512; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Associated Medical Specialists shall continue to provide professional internal medicine services, including cardiology; and

WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into the Amendment:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
