RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH STANFORD HOSPITALS AND CLINICS
FOR PHYSICIAN SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
|
WHEREAS, on September 9th the Board of Supervisors, by Resolution 66260, authorized an Agreement in the amount of $830,000 for the term from March 1, 2002 through June 30, 2003 for physician services to the San Mateo Medical Center; and
|
|
|
WHEREAS, the San Mateo Medical Center requires uninterrupted physician services, including gastroenterology, obstetrics and gynecology, internal medicine, and infectious diseases; and
|
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Stanford Hospitals and Clinics has provided physician services from the period of July 1, 2003 and will continue to do so through June 30, 2005; and
|
|
|
WHEREAS, the parties now wish to amend the previous Agreement by extending the length of the contract for two years from July 1, 2003 to June 30, 2005 with an increase in reimbursement of $461,047 from $830,000 to $1, 291,047; and
|
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Stanford Hospitals and Clinics shall continue to provide professional physician services; and
|
|
|
WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into the Amendment:

|
NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
