RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENTS WITH NORTHERN CLAIMS MANAGEMENT, INC. FOR WORKERS’ COMPENSATION CLAIMS ADMINISTRATION SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance a request to authorize the President to enter into an agreement with Northern Claims Management, Inc. for the administration of workers’ compensation claims; and

WHEREAS, said Northern Claims Management, Inc. wishes to enter into Agreement to provide the above-referenced services for the period from June 1, 2005 through May 31, 2008, with the right of the County, in its sole discretion, to extend the contract to May 31, 2009 or May 31, 2010 by exercising County reserved options; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into said Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED, that this Board of Supervisors authorizes its President to enter into said agreement with Northern Claims Management, Inc. in an amount not to exceed $2,022,654 for the three year term, plus $701,364 if the County exercises its fourth year option, plus $715,392 if the County exercises its fifth year option.
