RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AN AGREEMENT BETWEEN THE CHILDREN’S HEALTH INITIATIVE AND THE SAN MATEO HEALTH COMMISSION FOR THE PERIOD FEBRUARY 12, 2005 – FEBRUARY 11, 2008 AT AN AMOUNT NOT TO EXCEED $92.15 PER MEMBER PER MONTH

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there was agreement #065792 for the period January 28, 2003 to February 11, 2005; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and approval, an Agreement, reference to which is hereby made for further particulars, whereby the San Mateo Health Commission will continue to support the Children’s Health Initiative by administering health insurance medical, dental, and vision benefits for Healthy Kids eligible children from birth through age 18 years under a contract with the County of San Mateo;

WHEREAS, both parties now wish to enter into an Agreement, to provide health insurance to Healthy Kids members for the period of February 12, 2005 to February 11, 2008 and not to exceed $92.15 per member per month;

WHEREAS, this Board has been presented with Agreement and has approved it as to both form and content, and desires to enter into said Agreement;

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
