RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO EXECUTE AND AGREEMENT WITH AMERICAN MEDICAL RESPONSE (AMR) FOR PHLEBOTOMY SERVICES IN AN AMOUNT NOT TO EXCEED $216,000 FOR THE TERM OF JULY 1, 2005 – DECEMBER 31, 2006.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, parties wish to enter into an Agreement to provide for phlebotomy services for the period of July 1, 2005 – December 31, 2006 for total obligation not to exceed $216,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, an Agreement between the County of San Mateo and American Medical Response, reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same; and

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be and is hereby authorized and directed to execute said agreement for and on behalf of the County of San Mateo, and the Clerk shall attest for the President’s signature thereto.
