RESOLUTION AUTHORIZING THE PRESIDNT OF THE BOARD TO EXECUTE AN AMENDMENT TO THE AGREEMENT WITH AMERICAN MEDICAL RESPONSE WEST (AMR) EXTENDING THE TERM OF THE AGREEMENT TO PROVIDE MEDICAL AND NON-MEDICAL TRANSPORTATION OF SELECTED PATIENTS THROUGH DECEMBER 31, 2005 FOR AN ANNUAL AMOUNT OF $168,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on November 2, 1999 the Board of Supervisors, by Resolution No. 63173, authorized an agreement for an annual amount of $168,000 for the term of January 1, 1999 through December 31, 2004; and

WHEREAS, on October 8, 2002 the Board of Supervisors, by Resolution No. 065594, authorized a first amendment without a change to the term or amount, to clarify policies and procedures; and

WHEREAS, on December 14, 2004 the Board of Supervisors, by Resolution No. 067105, authorized an amendment to extend the term of the Agreement through March 31, 2005, with an annual amount of $168,000

WHEREAS, on there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment to the Agreement to extend the term to January 1, 1999 through December 31, 2005, with an annual amount of $168,000, reference to which is made for further particulars; and

WHEREAS, the Board has been presented with a form of the Amendment to this Agreement and has examined and approved it as to both form and content and desires to enter into the Amended Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be, and is hereby authorized and directed to execute said Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
