RESOLUTION 1) AUTHORIZING THE EXECUTION OF AN AMENDMENT TO THE PREVIOUS AGREEMENT WITH INFOIMAGE, INC.; 2) WAIVING THE REQUEST FOR PROPOSAL (RFP) PROCESS; 3) AUTHORIZING THE EXECUTION OF A NEW AGREEMENT WITH INFOIMAGE, INC., TO PROCESS, PRINT AND MAIL STATEMENTS FOR ENVIRONMENTAL HEALTH, MENTAL HEALTH AND SAN MATEO MEDICAL CENTER; AND 4) AUTHORIZING THE CHIEF EXECUTIVE OFFICER OF SAN MATEO MEDICAL CENTER TO EXECUTE AMENDMENTS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, On September 17, 2002, your Board approved an Agreement with InfoImage, Inc. for the term January 1, 2002 to April 30, 2005 with a maximum amount of $531,834.15; and

 

WHEREAS, restructuring of San Mateo Medical Center’s statement logic and an increase in the number of statements mailed to patients has depleted the maximum amount in the previous agreement. An Amendment increasing the maximum amount of the previous Agreement by $25,000 is necessary ; and

 

WHEREAS, the best interests of the County of San Mateo would be served by a waiver of the Request for Proposal (RFP). San Mateo Medical Center (SMMC), in collaboration with the San Mateo County Treasurer’s Office and San Mateo Medical Center’s Information Systems group, has implemented a program for automatic processing of patient payments. InfoImage has played a key role in designing and implementing this program. Changing vendors at this time will compromise what has been accomplished and force SMMC to redo a significant portion of work as a new vendor learns to catch up with SMMC processes; and

 

WHEREAS, the parties wish to enter into an Amendment to the previous Agreement with InfoImage by increasing the maximum amount by $25,000 changing the Agreement obligation from $531,834.15 to $556,834.15; and

 

WHEREAS, the parties wish to enter into a new Agreement with InfoImage to provide laser printing, mailing, and programming services to the Health Services Department and to San Mateo Medical Center, for the period May 1, 2005 to April 30, 2006, for a total obligation of $220,000;and

 

WHEREAS, the Board has been presented with an Amendment to the previous Agreement and a new Agreement between the County of San Mateo and InfoImage, Inc. and desires to enter into the same.

 

NOW THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be, and is hereby authorized and directed to execute said Amendment to the previous agreement and said new Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

IT IS HEREBY FURTHER RESOLVED THAT the Request for Proposal process is hereby waived; and

 

BE IT FURTHER RESOLVED that the Chief Executive Officer of San Mateo Medical Center shall be authorized to execute subsequent amendments increasing the County’s maximum fiscal obligation, provided however, that such authority is limited to increasing the County’s maximum fiscal obligation to an amount not to exceed $25,000.
