RESOLUTION AUTHORIZING A) EXECUTION OF AMENDMENT 2005-01 TO THE PRIMARY CARE PHYSICIAN MEDI-CAL SERVICES AGREEMENT WITH THE SAN MATEO HEALTH COMMISSION AND B) EXECUTION OF AMENDMENT 2005-01A TO THE HOSPITAL AGREEMENT WITH THE SAN MATEO HEALTH COMMISSION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance Amendment 2005-01 to the Primary Care Physician Medi-Cal Services Agreement and Amendment 2005-01A to the Hospital Agreement with the San Mateo Health Commission reference to which is hereby made for further particulars, whereby the San Mateo Health Commission will continue to pay for Medi-Cal reimbursable hospital and medical services provided to San Mateo County Medi-Cal recipients at San Mateo Medical Center;

WHEREAS, Amendment 2005-01 to the Primary Care Physician Agreement adjusts the Full Capitation Allocations to reflect the CFY 2005 budget Adjustments made by the Commission and modifies the risk sharing provisions of the Primary Care physician. Amendment 2005-01A to the Hospital Agreement amends the Full Capitation Allocations of the current Hospital Agreement to reflect the budget adopted by the San Mateo Health Commission for CFY 2005; and

WHEREAS, this Board has been presented with a form of the Amendments to the Agreements and has examined and approved it as to both form and content and desires to enter into the Amendments to the Agreements:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendments to the Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
