BACKGROUND:

San Mateo County is responsible for providing medical care to its medically indigent adult (MIA) population. This care includes acute inpatient dialysis treatment at San Mateo Medical Center (SMMC). Because of necessary equipment and specialized staffing, it is more economical to contract out for this service. After a Request for Proposal process in May 2003, Gambro was selected as the vendor for the provision of acute inpatient dialysis treatment. The term of the Agreement is July 1, 2003 to June 30, 2005, with a maximum amount of $400,000.

On July 27, 2004, your Board approved Resolution 66846 amending the agreement. The agreement was amended to institute a fee for an aborted procedure.

DISCUSSION:

The number of patients referred during the term of this agreement, as compared with historical data, has increased. This increased number of patients requiring dialysis as well as the condition of one SMMC psychiatric patient requiring acute dialysis three times weekly has contributed to depleting the maximum amount allowed in this contract. An increase of $37,000 is being requested to allow San Mateo Medical Center to pay pending invoices incurred due to this increased need for services.

A request for Proposal (RFP) for dialysis services is currently in process. A vendor will be chosen within the next two weeks and a new contract will be presented before your Board in July.

County Counsel and Risk Management have reviewed this Amendment.

FISCAL IMPACT:

This amendment increases the maximum amount of the Agreement by $37,000 from $400,000 to $437,000. Funds are included in SMMC’s FY2004-05 Adopted Budget. The term of the agreement remains the same, July 1, 2003 through June 30, 2005.
