RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO

A) EXECUTE AN AGREEMENT WITH COMMUNITY DENTAL CARE, INC. IN THE AMOUNT OF $198,774 TO PROVIDE HIV DENTAL SERVICES FOR THE PERIOD OF MARCH 1, 2005 TO FEBRUARY 28, 2006, AND B) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made for further particulars, whereby Community Dental Care, Inc., a California Nonprofit Corporation shall provide dental services to HIV/AIDS clients of the San Mateo County AIDS Program; and

WHEREAS, the term of said Agreement is March 1, 2005 through February 28, 2006, and the maximum dollar amount is $198,774; and

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Agreement with Community Dental Care, Inc., a California Nonprofit Corporation is hereby approved, and the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Agreement as approved by the County Manager and County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to execute minor amendments and modifications to the Agreement not to exceed $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
