RESOLUTION AUTHORIZING THE EXECUTION OF A SECOND AMENDMENT TO THE AGREEMENT WITH STANFORD UNIVERSITY TO CONDUCT A CLINICAL TRIAL ON CORONARY HEALTH PREVENTION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on October 17, 2003, the Purchasing Agent approved an Agreement with Stanford University to conduct a clinical trial on Coronary Health Prevention with San Mateo County patients to evaluate whether diet and counseling will lower the risk of patient heart disease. The term of the Agreement was September 1, 2003 to Mary 31, 2004, whereby Leland Stanford University allocated $92,991 for the provision of clinical trials at four County clinics; and

 

WHEREAS, on November 9, 2004, your Board adopted Resolution 67037 which authorized an Amendment to the Agreement whereby the term of the agreement was extended by one year to March 31, 2005 and the amount allocated was increased by $163,751.50;

 

WHEREAS, the parties wish to further amend the agreement by increasing the amount by $168,664, changing the Agreement maximum from $256,742.50 to $425,406.50, and extending the term from March 31, 2005 to March 31, 2006 to allow San Mateo Medical Center to complete its scope of work;

 

WHEREAS, this Board has been presented with the Amendment and desires to enter into same.

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said second Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
