RESOLUTION AUTHORIZING AMENDMENT II TO AGREEMENT NUMBER
HI-0405-08 WITH THE CALIFORNIA DEPARTMENT OF AGING FOR INCLUSION OF FUNDING FOR ASSISTANCE WITH IMPLEMENTING THE MEDICARE PRESCRIPTION DRUG, IMPROVEMENT, AND MODERNIZATION ACT OF 2003 INCREASING THE CONTRACT BY $10,632 TO $173,447 WITH NO CHANGE TO THE TERM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on August 31, 2004, the Board of Supervisors, by Resolution Number 66876, authorized an Agreement in the amount of $148,118 for the term of July 1, 2004 through June 30, 2005, for the purpose of providing a Health Insurance Counseling and Advocacy Program (HICAP);

WHEREAS, on March 9, 2005, the Director of the Health Department authorized Amendment Number 57000-05-D020-1 to increase federal funding by $14,697 for a total obligation of $162,815 for the inclusion of Bilingual and Linguistic Program services, with no change to the term of the current agreement;

WHEREAS, the parties now wish to amend the Agreement further by increasing the federal funding by $10,632 for a total obligation of $173,447 for assistance with implementation of the Medicare Prescription Drug, Improvement, and Modernization Act of 2003, with no change to the term; and

WHEREAS, the Board has been presented with Amendment II and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors is hereby authorized and directed to execute said Amendment II to the Agreement for and on behalf of the County of San Mateo and the Clerk of this Board shall attest the President’s signature thereto.
