RESOLUTION AUTHORIZING 1) AGREEMENTS WITH ALZHEIMER’S ASSOCIATION; AVENIDAS; CATHOLIC CHARITIES, CYO; CENTER FOR INDEPENDENCE OF THE DISABLED; CITY OF DALY CITY; CITY OF EAST PALO ALTO; CITY OF MENLO PARK; CITY OF PACIFICA; CITY OF SAN BRUNO; CITY OF SAN MATEO; CITY OF SOUTH SAN FRANCISCO; COASTSIDE ADULT DAY HEALTH CENTER; EDGEWOOD CENTER FOR CHILDREN AND FAMILIES; FAMILY SERVICE AGENCY; LEGAL AID SOCIETY; MILLS-PENINSULA SENIOR FOCUS; OMBUDSMAN OF SAN MATEO, INC.; PENINSULA VOLUNTEERS; SECOND HARVEST FOOD BANK; SENIOR COASTSIDERS; AND SEQUOIA HOSPITAL FOR THE PROVISION OF SUPPORTIVE, NUTRITION AND COMMUNITY-BASED SERVICES FOR THE TERM OF JULY 1, 2005 THROUGH JUNE 30, 2006, IN THE TOTAL AMOUNT OF $2,569,922; AND 2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THESE AGREEMENTS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to the Older Americans Act and the Older Californians Act;

WHEREAS, the parties wish to enter into the Agreements, to provide Adult Day Care/Adult Day Health Care, Alzheimer’s Day Program, Case Management, Brown Bag Program, Congregate Nutrition, Disease Prevention/Health Promotion, Falls Prevention Task Force, Family Caregiver Support Program Services, Home-Delivered Meals, Legal Assistance, Ombudsman Services, Senior Employment, and Transportation services to older adults and adults with disabilities, for the term of July 1, 2005 through June 30, 2006;

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, Agreements between the County of San Mateo and Alzheimer’s Association; Avenidas; Catholic Charities, CYO; Center for Independence of the Disabled; City of Daly City; City of East Palo Alto; City of Menlo Park; City of Pacifica; City of San Bruno; City of San Mateo; City of South San Francisco; Coastside Adult Day Health Center; Edgewood Center for Children and Families; Family Service Agency; Legal Aid Society; Mills-Peninsula Senior Focus; Ombudsman of San Mateo, Inc.; Peninsula Volunteers; Second Harvest Food Bank; Senior Coastsiders; and Sequoia Hospital, reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

IT IS HEREBY FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications not to exceed $25,000 and to make minor changes in the types of services and activities provided under these Agreements.
