RESOLUTION AUTHORIZING THE EXECUTION OF A FIRST AMENDMENT TO THE AGREEMENT WITH THE DEPARTMENT OF REHABILITATION

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, The Board of Supervisors authorized the execution of an Agreement issued by the Department of Rehabilitation by Resolution Number 067074 for the purpose of funding situational assessment, work adjustment, employment preparation, job development and placement services for County Mental Health consumers in the amount of $767,190, which provided funding to the Human Services Agency for FY 2004-05 services only, with funding for future years in the form of Amendments, and the term of said Agreement is July 1, 2004 through June 30, 2007; and

WHEREAS, the Department of Rehabilitation has issued a First Amendment to the Agreement for the purpose of adding funds for FY 2005-06 in the amount of $767,190 changing the Agreement obligation from $767,190 to $1,534,380 and adding additional monitoring and reporting provisions, to which the County agrees; and

WHEREAS, this Board has been presented with a form of this First Amendment to the Agreement and has examined and approved said First Amendment as to both form and content and desires to enter into same.

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said First Amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
