RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH ANLEE KUO, M.D. TO INCREASE THE MAXIMUM OBLIGATION BY $5,337 TO $105,186 WITH NO CHANGE IN THE TERM

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on September 1, 2004, the County Manager executed an agreement with Anlee Kuo, M.D. for a maximum obligation of $99,849 for the term July 12, 2004, through June 30, 2005; and

WHEREAS, the County and the Contractor now wish to amend the Agreement to increase the Agreement maximum obligation by $5,337 to $105,186 with no change in the term; and

WHEREAS, this Board has been presented with the amended Agreement and has examined and approved it as to both form and content and desires to enter into the amended Agreement;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby, authorized and directed to execute said amendment to the Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.
