RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH MEDIWARE INFORMATION SYSTEMS, INC. FOR HARDWARE AND SOFTWARE INSTALLATION, AND SUPPORT IN AN AMOUNT NOT TO EXCEED $231,429, AND GRANTING SIGNATURE AUTHORITY TO THE CHIEF INFORMATION OFFICER TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS IN AN AMOUNT NOT TO EXCEED $25,000.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, this Board has been presented with documentation of a formal Request for Proposal process which supports the selection of Mediware Information Systems, Inc. due to Mediware’s experience interfacing with existing Medical Center systems and their ability to serve as a single source for hardware and software installation, support, and FDA validation;

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement between the County and Mediware Information Systems, Inc. for hardware and software services to install and support a blood bank laboratory information system at San Mateo Medical Center for the period September 1, 2005 to August 31, 2008 with an amount not to exceed $231,429;

WHEREAS, this Board has been requested to grant signature authority to the Chief Information Officer to execute subsequent amendments and minor modifications to this Agreement in an amount not to exceed $25,000; and

WHEREAS, this Board has been presented with a form of an Agreement and said Board has examined and approved same as to both form and content and desires to enter into same;

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED

1) The Request for Proposal process for hardware and software installation and support is hereby approved; 2) Signature authority is granted to the Chief Information Officer to execute subsequent amendments and minor modifications in an amount not to exceed $25,000; 3) It is hereby authorized that the President of the Board of Supervisors is authorized and directed to execute said Agreement as is approved by the County Manager and the County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.
