RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD TO

A) EXECUTE AN AGREEMENT WITH THE SAN FRANCISCO REDEVELOPMENT AGENCY TO PROVIDE SHORT-TERM RENT AND MORTGAGE ASSISTANCE; CASE MANAGEMENT AND ATTENDANT CARE SERVICES IN THE AMOUNT OF $740,000 FOR THE PERIOD OF JULY 1, 2005 TO JUNE 30, 2006, AND B) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with the San Francisco Redevelopment Agency, reference to which is hereby made for further particulars, whereby San Mateo County AIDS Program shall provide short-term rent and mortgage assistance, case management and attendant care services; and

 

WHEREAS, the term of said Agreement is July 1, 2005 through June 30, 2006, and the maximum dollar amount is $740,000; and

 

WHEREAS, this Board has been presented with the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement.

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Agreement with the San Francisco Redevelopment Agency is hereby approved, and the President of this Board of Supervisors is hereby authorized and directed to execute the aforesaid Agreement as approved by the County Manager and County Counsel for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED, that the Director of the Health Department is hereby authorized to execute minor amendments and modifications to the Agreement not to exceed $25,000, and to make minor changes in the types of services and activities provided under the Agreement.
