BOARD OF SUPERVISORS, COUNTY OF SAN MATEO, STATE OF CALIFORNIA

RESOLUTION: (1) AUTHORIZING AGREEMENTS WITH CATHOLIC CHARITIES, CYO; MILLS-PENINSULA SENIOR FOCUS, INC.; PENINSULA VOLUNTEERS; AND THE CITY OF SOUTH SAN FRANCISCO ADULT DAY CARE PROGRAM; AND (2) AUTHORIZING THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THESE AGREEMENTS

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the parties wish to enter into the Agreements to provide adult day care services to Aging and Adult Services’ referred clients for the period of July 1, 2005 through June 30, 2009 in the collective amount of $141,600 for FY 2005-06;

WHEREAS, amounts to be paid for subsequent fiscal years will be determined at a later date and amendments will be brought before this Board of Supervisors;

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, Agreements between the County of San Mateo and Catholic Charities, CYO; Mills-Peninsula Senior Focus, Inc.; Peninsula Volunteers; and the City of South San Francisco Adult Day Care Program, reference to which is hereby made for further particulars, the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

IT IS HEREBY FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications not to exceed $25,000 and to make minor changes in the types of services and activities provided under these Agreements.
