RESOLUTION AUTHORIZING 1) THE EXECUTION OF AN AMENDMENT TO THE FY 2005-06 SOFTWARE SUBLICENSE AGREEMENT AND THE MAINTENANCE AND ENHANCEMENT AGREEMENT WITH THE PUBLIC HEALTH INSTITUTE ONE-E-APP PROGRAM OFFICE INCREASING THE AMOUNT BY $176,000, FROM $98,580 TO $274,580, FOR MODIFICATIONS TO CONDUCT FULL SCREENING OF APPLICANTS FOR HEALTH COVERAGE AND INSURANCE PROGRAMS FOR ADULTS AND CHILDREN, WITH NO CHANGE IN TERM; AND 2) THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, on July 22 2005 County, by contract number 55000-06-C051,entered into an agreement win the amount $98,580 for the term of July 1, 2005 thought June 30, 2006 for the purpose of installation, configuration, implementation, maintenance, enhancement and support services; and;

 

WHEREAS, this has been presented to this Board of Supervisors for its consideration and approval, an Amendment to the Agreement, reference to which is hereby made for further particulars, whereby the Public Health Institute will continue to provide installation, configuration, implementation, maintenance, enhancement and support services to the Health Department under a contract with the County of San Mateo, increasing the amount by $176,000 to $274,580, with no change in the term; and;

 

WHEREAS, this Board has been presented with this Amendment and has approved them as to both form and content, and desires to enter into said Amendment;

 

NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said amendment for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

 

BE IT FURTHER RESOLVED that the Director of the Health Department is hereby authorized to execute subsequent Amendments and minor modifications to this Agreement not to exceed the aggregate of $25,000 for the term of the Agreement provided the additional amount is within budgetary appropriations and the term is not extended.
