RESOLUTION A) ACCEPTING A GRANT FROM THE CAIFORNIA HEALTHCARE FOUNDATION (CHCF) IN THE AMOUNT OF $200,000 TO DEVELOP AND IMPLEMENT THE “IMPROVING ASTHMA CARE THROUGH PATIENT EMPOWERMENT” PROGRAM IN SAN MATEO MEDICAL CENTER (SMMC) CLINICS; AND B) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE CALIFORNIA HEALTHCARE FOUNDATION (CHCF) TO DEVELOP AND IMPLEMENT SAID PROGRAM FOR THE TERM DECEMBER 17, 2005 TO SEPTEMBER 1, 2006

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
WHEREAS, , there has been presented to this Board of Supervisors for its consideration and acceptance an Award, reference to which is hereby made for further particulars, whereby the California HealthCare Foundation will allocate funds in the amount of $200,000 for the provision of a project for to empower young patients with asthma at selected San Mateo Medical Center clinics to participate in managing their chronic disease; and

WHEREAS, asthma continues to be a widespread chronic disease in our community, primarily striking children and adolescents. Since much of the experience and control of this condition occurs between clinic visits, SMMC patient with asthma need the tools to partner with their medical provider in maintaining their health at home, at school and in the community; and

WHEREAS, in September 2004, the California HealthCare Foundation issued a Request for Proposals inviting applicants to address patient self-management through the Consumer Partnerships in Chronic Disease Care initiative. SMMC applied for this funding and was one of 137 agencies who was awarded a grant; and
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an grant, reference to which is hereby made for further particulars, whereby the California HealthCare Foundation is awarding San Mateo Medical Center $200,000 to develop and implement the “Improving Asthma Care through Patient Empowerment” program in SMMC clinics; and;

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with CHCF to develop and implement said program for the term December 17, 2004 to September 1, 2006, and
|

WHEREAS, this Board has been presented with the Award and the Agreement and desires to accept the award and enter into the Agreement.
|
|
NOW, THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be, and is hereby authorized and directed to accept said Award and execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
