RESOLUTION A) ACCEPTING A GRANT FROM THE BETTY AND GORDON MOORE FOUNDATION FOR $75,000 TO SUPPORT SAN MATEO MEDICAL CENTER’S PARTICIPATION IN THE FOUNDATION’S SAN FRANCISCO BAY AREA CHALLENGE GRANT PROGRAM FOR IMPLEMENTAITON OF INTERVENTIONS RECOMMENDED BY THE INSTITUTE FOR HEALTH IMPROVEMENT’S “100K LIVES CAMPAIGN” AND B) AUTHORIZING THE EXECUTION OF AN AGREEMENT WITH THE BETTY AND GORDON MOORE FOUNDATION FOR SAN MATEO MEDICAL CENTER TO IMPLEMENT SAID INTERVENTIONS FOR THE TERM

JUNE 13, 2005 TO JUNE 30, 2007

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, The Betty and Gordon Moore Foundation, through its Betty Irene Moore Nursing Initiative, supports efforts to “Improve nursing –related patient outcomes in acute care hospitals”; and

WHEREAS, In March, 2005 the Foundation created the San Francisco Bay Area Challenge Grant Program to speed implementation of patient safety related interventions in Bay Area hospitals; San Mateo Medical Center (SMMC) applied for a Challenge grant in April, 2004; and

WHEREAS, On June 13, 2005, the Betty and Gordon Moore Foundation awarded San Mateo Medical Center a Challenge grant in the amount of $75,000 to participate in the Program; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Award, reference to which is hereby made for further particulars, whereby the Betty and Gordon Moore Foundation will allocate funds in the amount of $75,000 to support SMMC’s participation in the Foundation’s San Francisco Bay Area Challenge Grant Program ; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with the Betty and Gordon Moore Foundation for SMMC to implement interventions identified in the program for the term June 13, 2005 to June 30, 2007; and

WHEREAS, this Board has been presented with the Award and the Agreement and desires to accept the award and enter into the Agreement.

NOW, THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be, and is hereby authorized and directed to accept said Award and execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signature thereto.
