RESOLUTION AUTHORIZING EXECUTION OF AN AMENDMENT TO THE AGREEMENT WITH DRS. HERBERT AND COHEN MEDICAL GROUP
FOR ANESTHESIOLOGY SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
|
WHEREAS, on February 24th, 2004 the Board of Supervisors, by Resolution 66519, authorized an Agreement in the amount of $1,722,240 for the term March 1, 2004 through May 31, 2005 for professional anesthesia services to the San Mateo County Medical Center; and
|
|
|
WHEREAS, on April 12th, 2005 the Board of Supervisors, by Resolution 66061, authorized an Amendment to the Agreement to extend the term from March 1, 2004 through May 31, 2005 to March 1, 2004 through August 31, 2005; and
|
|
|
WHEREAS, the San Mateo Medical Center requires an increase in the length of anesthesiology services for a transition period between anesthesiology providers during a Request for Proposal (RFP) process; and
|
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Drs. Herbert and Cohen Medical Group shall provide professional anesthesia services starting March 1; and
|
|
|
WHEREAS, the parties now wish to amend the previous Agreement by extending the length of the contract by two months from March 1, 2004 to August 31, 2005 to March 1, 2004 through October 31, 2005; and
|
|
|
WHEREAS there has been presented to this Board of Supervisors for its consideration and acceptance an Amendment, reference to which is hereby made, whereby Drs. Herbert and Cohen Medical Group. shall continue to provide professional anesthesiology services; and

|
|
WHEREAS, this Board has been presented with a form of the Amendment and has examined and approved it as to both form and content and desires to enter into the Amendment:

|
NOW THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Amendment for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
