RESOLUTION AUTHORIZING (1) EXECUTION OF AN AGREEMENT WITH
C.H. MACK, INC., FOR AN AUTOMATED CASE MANAGEMENT SYSTEM FOR $149,420 FOR TERM OF OCTOBER 1, 2005 THROUGH SEPTEMBER 30, 2006, AND (2) THE HEALTH DEPARTMENT DIRECTOR TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an agreement, reference to which is hereby made for further particulars, whereby, C.H. Mack, Inc., will provide an automated case management system for specific programs in Aging and Adult Services, including the automation of the InterRAI-HC uniform assessment tool; and

WHEREAS, both parties now wish to enter into said Agreement to provide a case management system and to develop the InterRAI-HC uniform assessment tool within the Q Continuum System for the period of October 1, 2005 through September 30, 2006 with the option to renew for two addition one-year terms, for a total obligation of $149,420:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board be, and is hereby authorized and directed to execute said Agreement with C.H. Mack, Inc., for and on behalf of the County of San Mateo, and the Clerk of the Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Health Department Director is hereby authorized to execute subsequent Amendments and minor modifications to this Agreement not to exceed the aggregate of $25,000 and to make minor changes in the types of services and activities provided under the agreement, for the term of the Agreement.
