RESOLUTION A) AUTHORIZING THE EXECUTION OF AGREEMENTS WITH DISCHARGE RESOURCE GROUP, PREFERRED HEALTHCARE REGISTRY, INC. AND SHC SERVICES, INC. TO PROVIDE OUTPATIENT, ACUTE AND LONG TERM CARE REHABILITATION SERVICES AT SAN MATEO MEDICAL CENTER AND ALL FACILITES; AND B) AUTHORIZING THE CHIEF EXECUTIVE OFFICER OF SAN MATEO MEDICAL CENTER TO EXECUTE AMENDMENTS NOT TO EXCEED $25,000

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance three Agreements, reference to which is hereby made for further particulars, whereby Discharge Resource Group, Preferred Healthcare Registry, Inc. and SHC Services, Inc. shall provide licensed/registered therapists to San Mateo Medical Center and all facilities;

WHEREAS, contracting with these three registries is necessary to maintain the broadest pool of therapy contractors and to provide coverage for mandated therapist positions that may remain vacant for extended periods of time;

WHEREAS, the parties wish to enter into Agreements with Discharge Resource Group, Preferred Healthcare Registry, Inc. and SHC Services, Inc, to provide licensed/registered therapists to San Mateo Medical Center and all facilities, for the period July 1, 2005 to June 30, 2006, for a total collective obligation of $101,000; and

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance, agreements between the County of San Mateo and Discharge Resource Group, Preferred Healthcare Registry, Inc. and SHC Services, Inc., reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same.

NOW, THEREFORE, IT IS HEREBY RESOLVED that the Board of Supervisors be and is hereby authorized and directed to execute said Agreements for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED, that the Chief Executive Officer of San Mateo Medical Center shall be authorized to execute Amendments not to exceed $25,000.
