RESOLUTION AUTHORIZING THE PURCHASING AGENT TO ENTER INTO A 60-MONTH PURCHASE AGREEMENT WITH MALLINCKRODT, INC. TO PROVIDE METHADOSE ORAL CONCENTRATE

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, the Methadone Clinic which opened on July 1, 2000, at the VA campus in Menlo Park, is licensed to see 330 patients at the clinic site; and

WHEREAS, the clinic, which currently dispenses Methadose Oral Concentrate to 266 clients per day, cannot operate without an uninterrupted supply of Methadose Oral Concentrate;

WHEREAS, in September 2005, San Mateo Medical Center sent a Request for Quote (RFQ) to three known providers of Methadose Oral Concentrate: Cebert Pharmaceuticals, Inc., Roxanne Laboratories, and Mallinckrodt. Only Mallinckrodt responded to the RFQ; and

WHEREAS, as a result of this Request for Quotes, Mallinckrodt will be awarded the agreement to provide Methadose Oral Concentrate for 60 months, from the date the agreement is signed with a maximum of $300,000 for the term of the agreement.

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors hereby authorizes the Purchasing Agent to enter into a purchase agreement to acquire Methadose Oral Concentrate for and on behalf of the County of San Mateo.
