RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH YOUTH AND FAMILY ENRICHMENT SERVICES (YFES) TO PROVIDE MENTAL HEALTH SERVICES, COMMUNITY WORKER SERVICES, AND SUPPORT FOR THE YOUTH DEVELOPMENT INITIATIVE FOR THE TERM OF JULY 1, 2005 THROUGH JUNE 30, 2006, IN THE AMOUNT OF $766,044

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement with Youth and Family Enrichment Services, reference to which is hereby made for further particulars; and

WHEREAS, both parties now wish to enter into an Agreement to provide mental health services through the Mental Health Plan, community worker services through the Public Health Pre-to-Three Program, and support for the Youth Development Initiative for the period of July 1, 2005 through June 30, 2006, for a total obligation of $766,044; and

WHEREAS, the Board of Supervisors has examined and approved the Agreement as to form and content and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

BE IT FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications not to exceed $25,000 and to make minor changes in the types of services and activities provided under the agreement.
