RESOLUTION AUTHORIZING 1) THE EXECUTION OF AGREEMENT WITH SELF-HELP FOR THE ELDERLY FOR THE PROVISION OF SUPPORTIVE, NUTRITION AND HEALTH INSURANCE COUNSELING AND ADVOCACY PROGRAM (HICAP) SERVICES FOR THE TERM OF JULY 1, 2005 THROUGH JUNE 30, 2006, IN THE TOTAL AMOUNT OF $376,558; AND 2) THE DIRECTOR OF THE HEALTH DEPARTMENT TO EXECUTE SUBSEQUENT AMENDMENTS AND MINOR MODIFICATIONS NOT TO EXCEED $25,000 AND TO MAKE MINOR CHANGES IN THE TYPES OF SERVICES AND ACTIVITIES PROVIDED UNDER THE AGREEMENT

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

 

WHEREAS, this Board has designated itself as the Area Agency on Aging of San Mateo County to carry out programs pursuant to the Older Americans Act;

 

WHEREAS, the parties wish to enter into the Agreement for the provision of case management, congregate nutrition, the Health Insurance Counseling and Advocacy Program (HICAP), and home-delivered meal services to older adults and adults with disabilities for the term of July 1, 2005 through June 30, 2006 in the amount of $376,558; and

 

WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement between the County of San Mateo and Self-Help for the Elderly, reference to which is hereby made for further particulars, and the Board of Supervisors has examined and approved the same as to form and content and desires to enter into the same:

 

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President’s signature thereto.

 

IT IS HEREBY FURTHER RESOLVED that the Director of the Health Department shall be authorized to execute subsequent amendments and minor modifications not to exceed $25,000 and to make minor changes in the types of services and activities provided under the Agreement.
