RESOLUTION AUTHORIZING THE PRESIDENT OF THE BOARD OF SUPERVISORS TO EXECUTE THE FIRST AMENDMENT TO THE AGREEMENT WITH OMBUDSMAN SERVICES OF SAN MATEO COUNTY, INC. (OSSMC), INCREASING THE TOTAL AMOUNT BY $75,000 TO $293,581 FOR THE TERM JULY 1, 2005 THROUGH JUNE 30, 2006.

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that

WHEREAS, on July 26, 2005, the Board of Supervisors, by Resolution 67461, authorized an Agreement in the amount of $218,581 for the term of July 1, 2005 through June 30, 2006 for the provision of ombudsman services in accordance with the California Department of Aging and/or state licensing regulations and the standards and requirements established by Aging and Adult Services of San Mateo County;

WHEREAS, the parties wish to amend the Agreement by increasing the amount by $75,000 changing the Agreement obligation from $218,581 to $293,581 with no change to the term, for the inclusion of investigation and monitoring of residential care facilities for individuals under 60 years of age and enhanced services to individuals 60 years of age or older residing in a long-term care facility; and

WHEREAS, the Board has been presented with this First Amendment and desires to enter into same:

NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors is hereby authorized and directed to execute said First Amendment to the Agreement for and on behalf of the County of San Mateo; and the Clerk of this Board shall attest the President’s signature thereto.
