RESOLUTION AUTHORIZING EXECUTION OF AN AGREEMENT WITH
SEQUOIA ANESTHESIA CONSULTANTS FOR
ANESTHESIOLOGY AND PAIN MANAGEMENT SERVICES

RESOLVED, by the Board of Supervisors of the County of San Mateo, State of California, that
|
|
|
WHEREAS, on February 24th, 2004 the Board of Supervisors, by Resolution 66519, authorized an Agreement with Drs. Herbert and Cohen Medical Group for anesthesiology services for the term March 1, 2004 through May 31, 2005 for anesthesia services to the San Mateo Medical Center; and
|
|
|
WHEREAS, on April 12th, 2005 the Board of Supervisors, by Resolution 67257, authorized an Amendment to the Agreement to extend the term through August 31, 2005; and
|
|
|
WHEREAS, on September 20th , 2005 the Board of Supervisors, by Resolution 67565, authorized an Amendment to the Agreement to extend the term through October 31, 2005 while a Request for Proposal (RFP) for a new anesthesiology group was in process; and
|
|
|
WHEREAS, the San Mateo Medical Center selected Sequoia Anesthesia Consultants to replace Drs. Herbert and Cohen Medical Group as the provider of uninterrupted anesthesiology and pain management services; and
|
|
|
WHEREAS, the parties now wish to enter into an Agreement for the term of November 1, 2005 to October 31, 2006 with a maximum reimbursement amount of $1,660,000; and
|
|
|
WHEREAS, there has been presented to this Board of Supervisors for its consideration and acceptance an Agreement, reference to which is hereby made, whereby Sequoia Anesthesia Consultants shall provide professional anesthesiology and pain management services; and

|
WHEREAS, this Board has been presented with a form of the Agreement and has examined and approved it as to both form and content and desires to enter into the Agreement:

|
NOW, THEREFORE, IT IS HEREBY DETERMINED AND ORDERED that the President of this Board of Supervisors be, and is hereby authorized and directed to execute said Agreement for and on behalf of the County of San Mateo, and the Clerk of this Board shall attest the President's signatures thereto.
