Background

There has been a shortage of adult and child psychiatrists in California and in the nation for the past several years. Mental Health has found it extremely difficult to fill adult and child psychiatrist positions. Physicians are exempt from the requirement for a Request for Proposals process, and Mental Health Services Division runs advertisements for specialty psychiatric services on a regular basis in the Northern California Psychiatric Newsletter and other venues as needed. Dr. Garg was selected from candidates who responded to those advertisements.

Mental Health Services has contracted with Dr. Garg since 1998.

Discussion

Dr. Garg will provide child psychiatric services at Central County Mental Health, Child Welfare, Receiving Home, Therapeutic Day Schools, and Tower House. The services shall include child and youth psychiatric evaluation and treatment, psychotherapy, family therapy and consultation, case consultation to staff, and other duties as assigned.

The term of this agreement is July 1, 2005 through June 30, 2007. The agreement has been reviewed and approved by County Counsel and Risk Management. The agreement with Dr. Garg is currently on the Continuing Resolution.

Fiscal Impact

Contract physicians receive the same payment scale as was negotiated by the Independent Practice Association (IPA). A 3% COLA has been included for FY 2005-06 and a 4% COLA has been included for FY 2006-07.

The contract maximum is $356,460 for the two-year term. The maximum for FY 2005-06 is $174,736 which has been included in the approved FY 2005-06 budget for Mental Health Services. State and federal Medi-Cal is expected to fund $89,115 of the cost. A total of $3,145 is funded through IDEA for AB 3632, and $20,968 is funded through the Human Service Agency’s out-of-home placement fund, a Children’s System of Care Match. Sales tax provides $59,411 through realignment, and the remaining $2,097 is Net County Cost.

The maximum for FY 2006-07 is $181,724. State and federal Medi-Cal is expected to fund $92,679, and $21,807 will be funded through the Human Service Agency’s out-of-home placement fund. A total of $3,271 will be funded through IDEA for AB 3632, $61,786 will be funded through sales tax provided through realignment, and $2,181 will be the Net County Cost.
