SUBJECT:

Agreement with Jeanette Bland, M.D.

 

Recommendation

Adopt a resolution:

1) Authorizing the President of the Board to execute an agreement with Jeanette Bland, M.D. for the provision of psychiatric services for the period July 1, 2005 through June 30, 2007, for a maximum obligation of $271,855, Net County Cost of $3,695; and

 

2) Authorizing the Director of Health to execute subsequent amendments and minor modifications not to exceed $25,000 in aggregate and to make minor changes in the types of services and activities provided under the Agreement.

The agreement contributes to this commitment and goal by providing psychiatric assessment and treatment services to adults at the South County Mental Health Clinic and psychiatric treatment services to youth at the Community Counseling Center in East Palo Alto.

*This is the first year collecting satisfaction data for contract psychiatrists. Mental Health Services will monitor contractor performance through the agreement term.

 

Background

There has been a shortage of adult and child psychiatrists in California and in the nation for the past several years. Mental Health has found it extremely difficult to fill adult and child psychiatrist positions. Physicians are exempt from the requirement for a Request for Proposals process. Mental Health Services Division runs advertisements for specialty psychiatric services on a regular basis in the Northern California Psychiatric Newsletter and other venues as needed. Dr. Bland was selected from candidates who responded to those advertisements.

 

The County Manager approved an agreement with Dr. Bland for the term January 1, 2005 through June 30, 2005.

 

Discussion

Dr. Bland will provide psychiatric assessment services to clients at the South County Mental Health Clinic and psychiatric treatment services for the youth at the Community Counseling Center located in East Palo Alto.

 

The term of this agreement is July 1, 2005 through June 30, 2007. The agreement has been reviewed and approved by County Counsel and Risk Management. The agreement with Dr. Bland is currently on the Continuing Resolution.

 

Fiscal Impact

Contract physicians receive the same payment scale as was negotiated by the Independent Practice Association (IPA). A 3% COLA has been included for FY 2005-06 and a 4% COLA has been included for FY 2006-07.

 

The contract maximum is $271,855 for the two-year term. The agreement maximum for FY 2005-06 is $133,259 which has been included in the approved FY 2005-06 budget for Mental Health Services. State and federal Medi-Cal will fund $99,673, Medicare will fund $399 and $2,401 will be funded by the State allocated IDEA funding for AB 3632 services. Sales tax provided through realignment will fund $28,975, and the remaining $1,811 is Net County Cost for unfunded AB 3632 services and County match for EPSDT.

 

The agreement maximum of FY 2006-07 is $138,596, which is included in the budget for FY 2006-07. State and federal Medi-Cal will fund $103,670, Medicare will fund $416 and $2,495 will be funded by the State allocated IDEA funding for AB 3632 services. Sales tax provided through realignment will fund $30,131, and the remaining $1,884 is Net County Cost for unfunded AB 3632 services and County match for EPSDT. The total Net County Cost for the two-year agreement is $3,695.
