Agreement with Lisa Nelson, M.D.

 

Recommendation

Adopt a resolution:

1)

Authorizing the President of the Board to execute an agreement with Lisa Nelson, M.D. for the provision of psychiatric services for children and youth for the term July 1, 2005 through June 30, 2007, for a maximum obligation of $237,640, Net County Cost of $39,923; and

   

2)

Authorizing the Director of Health to execute subsequent amendments and minor modifications not to exceed $25,000 in aggregate and to make minor changes in the types of services and activities provided under the Agreement.

Background

There has been a shortage of adult and child psychiatrists in California and in the nation for the past several years. Mental Health has found it extremely difficult to fill adult and child psychiatrist positions. Physicians are exempt from the requirement for a Request for Proposals process, and Mental Health Services Division runs advertisements for specialty psychiatric services on a regular basis in the Northern California Psychiatric Newsletter and other venues as needed. Dr. Nelson was selected from candidates who responded to those advertisements.

 

Mental Health Services has contracted with Dr. Nelson since September, 1998.

 

Discussion

Dr. Nelson will provide psychiatric services for children and youth in San Mateo County. The services shall include child and youth medication evaluation and treatment, diagnostic evaluation, psychotherapy, family consultation, treatment plan review, and case consultation.

 

The term of this agreement is July 1, 2005 through June 30, 2007. The agreement has been reviewed and approved by County Counsel and Risk Management. The agreement with Dr. Nelson is currently on the Continuing Resolution.

 

Fiscal Impact

Contract physicians receive the same payment scale as was negotiated by the Independent Practice Association (IPA). A 3% COLA has been included for FY 2005-06 and a 4% COLA has been included for FY 2006-07.

 

The contract maximum is $237,640 for the two-year term. The agreement maximum for FY 2005-06 is $116,490 which has been included in the Mental Health Services approved FY 2005-06 budget. State and federal Medi-Cal will fund $51,256 of the cost; $29,356 is funded through IDEA for AB 3632. A total of $16,308 will be funded by sales tax through realignment, and the remaining $19,570 is Net County Cost.

 

The agreement maximum for FY 2005-06 is $121,150. State and federal Medi-Cal is expected to fund $53,306 of the cost; $30,530 is expected to be funded through IDEA for AB 3632; $16,961 is from sales tax through Realignment; and the remaining $20,353 is Net County Cost. The total Net County Cost for the two-year agreement is $39,923.
