AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND BAY AREA UROLOGY GROUP FOR UROLOGY SERVICES
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and Bay Area Urology Group, hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, the parties entered into an Agreement 66774 for Urology services on June 29, 2004; and
WHEREAS, the parties wish to amend the Agreement to extend the term of the contract for an additional two months;
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
1.	Section 3.1 of the agreement is amended to read as follows: “This Agreement shall commence on June 1, 2004, (the "Effective Date") and shall continue for twenty one months. Unless sooner terminated, this Agreement shall expire and be of no further force and effect as of the end of business on the 28th day of February 2006.”
2.	Schedule B of the Original Agreement is hereby amended to read as follows: Year I (Monthly = $20,806.96) June 1, 2004 through August 31, 2004: $62,420.88 September 1, 2004 through November 30, 2004: $62,420.88 December 1, 2004 through February 28, 2005: $62,420.88 March 1, 2005 through May 31, 2005: $62,420.88 June 1, 2005 through August 31, 2005: $62,420.88 September 1, 2005 through November 30, 2005: $62,420.88 December 1, 2005 through February 28, 2006: $62,420.88
3.	All other terms and conditions of the agreement dated June 29, 2004, between the County and Contractor shall remain in full force and effect.
