AMENDMENT TO AGREEMENT BETWEEN THE COUNTY OF SAN MATEO AND JOHN MCNAMARA, M.D. FOR OPHTHALMOLOGY SERVICES
THIS AMENDMENT TO THE AGREEMENT, entered into this _____ day of _______________, 20_____, by and between the COUNTY OF SAN MATEO, hereinafter called "County," and John McNamara, M.D., hereinafter called "Contractor";
W I T N E S S E T H:
WHEREAS, pursuant to Government Code, Section 31000, County may contract with independent contractors for the furnishing of such services to or for County or any Department thereof;
WHEREAS, the parties entered into an Agreement for Ophthalmology services on October 19, 2004; and
WHEREAS, the parties wish to amend the Agreement to increase the maximum amount of the contract to reflect the increased level of service and productivity being provided by Contractor;
NOW, THEREFORE, IT IS HEREBY AGREED BY THE PARTIES HERETO AS FOLLOWS:
1.	Schedule B of the Original Agreement is hereby amended to read as follows: Total payment for services under this agreement will not exceed TWO HUNDRED THIRTY ONE THOUSAND THREE HUNDRED THIRTY EIGHT DOLLARS AND SEVENTY SIX CENTS ($231,338.76).
2.	All other terms and conditions of the agreement dated October 19, 2004, between the County and Contractor shall remain in full force and effect.
