SUBJECT:

Acceptance of a Grant Award from the U.S. Department of Health & Human Services (DHHS) Administration on Aging (AoA)

 

RECOMMENDATION:

Adopt a Resolution authorizing an Appropriation Transfer Request in the amount of $60,000 for Services and Supplies for the continued implementation of a uniform assessment tool and automated case management software system across Aging and Adult Services.

 

Commitment: Ensuring basic health and safety for all.

Goal(s) People – Help vulnerable people – the aged, disabled, mentally ill, at-risk youth and others – achieve a better quality of life.

 

This grant will contribute to this commitment and goal by funding part of the development cost of a uniform assessment tool that will be integrated within Aging and Adult Services’ new case management software application. This will enable Aging and Adult Services to provide integrated and consistent care to clients and to access timely and accurate programmatic data.

BACKGROUND:

In September of 2003, the California Legislature passed AB786 which designated San Mateo County as the State’s pilot for implementing a single uniform assessment tool for all in-home and community-based long-term care services. Using a single assessment tool across all AAS programs will allow AAS to standardize the content and quality of assessments, track client outcomes across time, track changes in the AAS client population, provide aggregate data for policy making and planning, and provide a more standardized response to client needs.

 

On September 29, 2005, AAS was awarded a grant from DHHS, in the amount of $60,000. The County Manager approved the grant on October 20, 2005. The grant is a cooperative agreement in which AAS will continue to work with AoA and to share innovations and best practices with the other grantees.

 

DISCUSSION:

This grant will assist in funding the continued implementation of a uniform assessment tool and automated case management software system across Aging and Adult Services.

 

FISCAL IMPACT:

The term of the grant is October 1, 2005 through September 30, 2006. The total award is $60,000. There is no Net County Cost as a result of this action.
