SUBJECT:

Acceptance of the Department of Health and Human Services (DHHS) Center for Substance Abuse Treatment (CSAT) Grant Number 1 H79 TI 17317-01

 

RECOMMENDATION:

A) Adopt a Resolution:

1)

Accepting a thirty six (36) month grant from the DHHS/CSAT to provide outpatient alcohol and drug treatment programs for Methamphetamine and other stimulant abuse on the rural Coastside of San Mateo County, in the amount of $1,196,055 for the period of August 15, 2005 to August 14, 2008; and

   

2)

Waiving the Request for Proposal (RFP) process for the services funded by the grant and authorizing an Agreement with El Centro de Libertad in the amount of $700,000 for contracted services funded by the grant, for the term of August 15, 2005 to February 14, 2009; and

   

3)

Authorizing the Director of the Human Services Agency to:

a) Execute subsequent Amendments and minor modifications not to exceed an aggregate of $25,000, and

b) Make changes in the types of services and activities provided by the Contractor, provided that the changes have no impact on the County’s maximum fiscal obligation.

   

B)

Adopt a Resolution Authorizing an Appropriation Transfer Request in the amount of $336,935.

   

C)

Adopt a Salary Ordinance Amendment adding one Human Services Analyst position to provide coordination as specified in the grant application.

   

VISION ALIGNMENT:

Commitment: Ensure basic health and safety for all.

Goals Number 8: Help vulnerable people – the aged, disabled, mentally ill, at-risk youth and others – achieve a better quality of life. The grant from the DHHS Center and the Agreement with El Centro de Libertad will contribute to this commitment and goal by assisting the rural Coastside population with methamphetamine and stimulant drug treatment services. Therefore, providing a healthier and safer community for all.

 

PERFORMANCE MEASURE (S):

Evaluation of the thirty six (36) month project will be conducted by the University of California San Francisco to assess treatment effectiveness through use of the Addiction Severity Index (ASI), random urinalysis result, client retention data, California Alcohol and Drug Data System (CADDS) and CSAT’s Government Programs Reporting Act (GPRA) tools.

Motivational Enhancement Groups

50% of new referrals will participate in Motivational Enhancement Groups and will progress within the Stages of Change model.

Family Education Groups

56% of unduplicated individuals referred for Outpatient Services will attend and bring family members or significant persons to Family Education Groups.

Continuing Care

58% of individuals completing Primary Care with a focus on methamphetamine abuse will participate in Continuing Care.

 

BACKGROUND:

In response to a Federal grant funding announcement, interested community organizations were invited to participate in a series of planning sessions to design a collaborative effort to provide Methamphetamine and other stimulant drug treatment for Coastside residents. The Human Services Agency, Alcohol and Other Drug Services (AOD) in collaboration with the El Centro de Libertad, and the University of California San Francisco (UCSF), submitted an application to the DHHS/CSAT for a thirty six (36) month grant, to provide specific Methamphetamine and other stimulant drug treatment for residents of the rural and Coastside areas of San Mateo County. These agencies have demonstrated an ability to provide services to the rural population of San Mateo County and are committed to participating in the implementation of this project.

 

DISCUSSION:

On August 26, 2005, the Human Services Agency received notification of a grant award in the amount of $1,196,055 for the period of August 15, 2005, to August 14, 2008. The grant project will expand services on the Coastside in three (3) critical evidence based practice areas: Motivational Enhancement Therapy (MET), Cognitive Behavioral Therapy (CBT) and Outpatient Treatment. Through contracts with the collaborating treatment provider, El Centro de Libertad, trained substance abuse counselors will be placed at Coastside sites. The counselors will provide assessment, case management and individual and group substance abuse treatment utilizing CBT and MET to a total of one hundred seventy four (174) men and women over the life of the grant. Ninety-six (96) families will participate in family education groups and seventy eight (78) people will participate in continuing care activities.

A Human Services Analyst will act as project coordinator and serve as liaison to the substance abuse treatment provider, supervise and monitor data collection, convene meetings, provide outreach support efforts, coordinate follow-up data collection, coordinate effects with the evaluators, maintain project records and prepare project reports.

The Appropriation Transfer Request recognizes the unanticipated revenue from DHHS/CSAT and appropriates the grant funds for the El Centro de Libertad contract and the additional staff position.

The RFP waiver is being requested because El Centro de Libertad collaborated with the Human Services Agency in writing the proposal that received this grant, and is identified in the grant as a partner.

The Agreement was delayed due to the length of time needed in negotiating program and contract deliverables.

The Agreement has been reviewed and approved as to form by the County Counsel’s Office. Risk Management has reviewed and approved the Contractor’s insurance coverage.

 

This grant is for a thirty six (36) month period beginning August 15, 2005 and ending August 14, 2008. Additional resources past this funded period have not yet been identified. AOD along with El Centro de Libertad will pursue additional resources in an attempt to sustain services. Possible funding sources may include Local/State/Federal agencies and Foundations. If the State Block Grant Funds are increased, they may be utilized for sustaining substance abuse treatment services that began under this grant. However, due to economic uncertainties, these efforts may not result in additional funding. If no additional resources are identified the program will cease on August 14, 2008. This Agreement is for a forty two (42) month period beginning August 15, 2005 and ending February 14, 2009. The additional unpaid time (6 months) is needed after the grant term ends for the provider to complete the required reporting/narrative to AOD.

 

FISCAL IMPACT:

The thirty six (36) month grant period is from August 15, 2005 to August 14, 2008. The Agreement with El Centro de Libertad is for the period of August 15, 2005 to February 14, 2009, in the amount of $700,000, and is fully funded under this grant. The remainder of these funds will be used for salaries and benefits, supplies, travel, evaluation services, and future contracted services. Approval of the ATR will amend the Human Services Agency’s FY 2005-06 Budget to create sufficient appropriations to cover anticipated cost within the current fiscal year. Appropriations for future years will be included in the Recommended Budgets for FY 2006-07, FY 2007-08 and FY 2008-09. There is no Net County Cost.
