Authorizing the execution of an Agreement with Ravenswood Family Health Center to provide primary care services to San Mateo County’s homeless population, for the term of November 1, 2005 to October 31, 2006, in the amount of $108,000.

San Mateo Medical Center was awarded the Health Care for the Homeless (HCH) grant from the federal Bureau of Primary Health Care of the Health and Resource Services Administrative to provide primary care, dental care, outreach, health education, case management, and supportive services to the San Mateo County’s homeless population.
HCH is designed to assist homeless adults, families and youth with access to and use of the San Mateo Medical Center’s continuum of primary care, dental care, mental health, substance abuse, and other support services.
In Winter 2003, the HCH Advisory Committee developed a strategic plan for the HCH Program. The goals of the Strategic Plan include: 1) Serve a large portion of the homeless population who are on the street or in shelters; 2) Provide one or more of the following services to homeless individuals: primary care services, dental care services, substance abuse and mental health treatment; 3) eliminate barriers and provide better access to services by outreaching to homeless individuals.
A Request for Proposal process followed and based on the priorities of the strategic plan, the HCH Advisory Committee chose to continue to fund five current contractors and provide funding to one new agency. Ravenswood was awarded $30,597 in HCH funding for FY 2004-05 to provide primary care services to 300 homeless individuals for the term November 1, 2004 to October 31, 2005. In August of 2005, the contract was amended to provide Ravenswood a supplementary $68,400 to serve an additional 300 homeless individuals.

During the term of the previous agreement, Ravenswood well exceeded the contract objectives and goals. The San Mateo Medical Center Board of Directors approved awarding Ravenswood with $108,000 to provide primary care services to 700 homeless individuals for FY2005-06.
Since the last RFP for these services was conducted only last year, SMMC is requesting a waiver of the RFP process.
County Counsel and Risk Management have both reviewed and approved this Agreement.

The term of this Agreement is November 1, 2005 through October 31, 2006. The maximum amount of this Agreement is $108,000. All funds for this Agreement are provided by the HCH grant. Expenditures and grant revenue for this Agreement are included in San Mateo Medical Center’s FY2005-06 Adopted Budget. There is no Net County Cost.
