Amendment to the Agreement with John McNamara, MD for Ophthalmology Services

 

RECOMMENDATION:

Adopt a resolution authorizing the President of the Board to execute an Amendment to the Agreement with John McNamara, MD to provide ophthalmology services increasing the maximum Agreement obligation by $115,669.38 from $115,669.38 to $231,338.76.

BACKGROUND:

Dr. John McNamara has provided ophthalmology services to the patients of San Mateo Medical Center for the last several years. In particular, these services include the diagnosis and treatment for diseases of the eye, including glaucoma surgery, cataract surgery, and clinics for eye injury, trauma, or refractive corrections. Patients treated at SMMC have a continuous need for such eye care services and Dr. McNamara offers the hospital the benefits of an experienced provider familiar with the complex nature of the County’s patient base.

 

DISCUSSION:

In 2004 eye injury, trauma, disease, and surgery services for San Mateo Medical Center were contracted among four ophthalmologists. One of the contractors terminated his contract with the County in January 2005. Another reduced their availability for services in April 2005. Dr. John McNamara increased his availability to cover the service gap such that his current productivity levels doubled in volume.

The original Agreement budget was based on an annual productivity level of 967.27 RBRVS work units with a cushion to cover anticipated increases in surgical cases. Actual and projected productivity levels for the contractual period are anticipated at 3.5 times the original amount to reflect Dr. McNamara’s assumption of the terminated contractor’s workload. Funds budgeted for the contract terminated in January 2005 are being shifted to cover the increased financial obligation for this contract.

Ophthalmology services are provided under the general direction of the Chief of Medicine. Monitoring and evaluation of the quality of services provided by contractor are accomplished through peer review by the Quality Assurance Committee and the Executive Committee of the medical staff.

 

FISCAL IMPACT:

The Agreement is being amended to increase the maximum contract amount by $115,669.38. The maximum amount the County shall be obligated to pay Contractor is increased from $115,669.38 to $231,338.76 Funds were included in the San Mateo Medical Center’s FY2004-05 Adopted Budget and are included in the FY2005-06 Adopted Budget.
